Sports Physical Requirement

All QMHS athletes must have a sports physical each school year from May of the current school year through June 30 of the succeeding year in order to try out for QMHS athletics. A copy of the completed physical form should be returned to the coach or school nurse. You can pick up a form in the main office or go to the VISAA.org link provided here. Please make a copy of the completed sports physical for your records before you turn it in to the school.
